Complete the Disaster Recovery Self-Assessment (DRSA) to check in with your wellness! You’ll receive a free, downloadable report with your wellness score to tell you where you are thriving and areas that need more attention.

Completing this brief, individualized assessment will help you reflect on six wellness dimensions important for overall health and may increase your awareness of aspects of your wellness that you have not considered previously. It will also offer practical strategies to address your specific needs associated with disaster recovery.

Holly Hatton-Bowers and Lorey Wheeler in partnership with the Communications, Web and Technology team at the Nebraska Center for Research on Children, Youth, Families & Schools. The assessment, as it is currently developed, is not a validated tool and is not intended for research or diagnostic purposes. The DRSA is intended for individual self-reflection and action planning.

By answering the DRSA, you agree to participate in the Reach Out for Wellness project. Data gathered through the DRSA will remain anonymous. We will never use your name or other personally identifiable information in summarized findings that are shared with the funding agency and the general public.
